Roles & Responsibilities

• Consult with patients to understand their physical condition, symptoms, and medical history
• Diagnose movement dysfunction through observation and patient interaction
• Develop individualized treatment plans to meet each patient's goals
• Coach patients on therapeutic exercises and stretches to improve mobility and strength
• Use hands-on therapy techniques like massage to ease pain and promote healing
• Maintain detailed records of patient conditions, care plans, and progress
• Collaborate with speech therapists, physical therapists, and other care providers for the best outcomes
• Report progress to families, providers, and care team members while maintaining confidentiality

Qualifications

• Current Physical Therapist state license (must be actively held)
• Current BLS certification or ability to obtain within 6 months of hire
• Experience working directly with patients in a long-term care or hospital setting
• Strong documentation and verbal communication skills
• Ability to work collaboratively as part of a patient care team

Education

• Masters Degree from an accredited physical therapy program
