Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in their home environment
Develop and update individualized plans of care in collaboration with physicians
Provide therapeutic exercises, gait training, and mobility interventions
Instruct patients and families on use of therapeutic appliances and home exercise programs
Document patient progress and communicate changes to the care team
Supervise home health aides and participate in case conferences
Comply with Medicare, Medi-Cal, and OSHA regulations

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Valid Physical Therapy license in California
Excellent communication and organizational skills
Comfortable traveling throughout Yolo County and surrounding areas

Education

Graduate of an accredited Physical Therapy program

Knowledge & Skills

Strong assessment and clinical reasoning abilities
Knowledge of home health regulations and documentation requirements
Ability to work independently and manage a caseload
Proficiency in electronic health records
