Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; occasional Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' therapeutic/rehabilitative status
Develop and update total plan of care in collaboration with physician
Instruct patients and families in use of therapeutic appliances
Report patient progress to physician and document in health record
Supervise home health aides when participating in patient's rehabilitation
Prepare clinical and progress summaries based on goal attainment

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification
Excellent oral and written communication skills
Strong organizational skills

Knowledge & Skills

Knowledge of Title 22 Medicare Conditions of Participation, Medi-Cal, and OSHA regulations
Skilled in physical therapy assessment and treatment planning
Ability to work independently in home health settings
Experience with electronic documentation and case management
