Contract Type: Travel Contract

Shift Type: Monday - Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to home health patients in their own residences
Develop and implement individualized care plans to improve mobility and function
Collaborate with an interdisciplinary team to ensure comprehensive patient care
Document patient progress accurately using HomeCare/HomeBase EMR
Educate patients and families on therapeutic exercises and safety at home

Qualifications

Active North Dakota Physical Therapy License required
BLS certification from the American Heart Association
Valid driver's license and auto insurance
Minimum 1 year of home health experience
Ability to travel within a 60-mile radius of Valley City

Knowledge & Skills

Expertise in physical therapy for post-surgery, fall recovery, and chronic conditions
Experience with home health and hospice patient populations
Proficiency with HomeCare/HomeBase EMR system
Strong communication and time management skills
