Education
- CA PT license"
- Other required certifications: Valid driver's license, car insurance
- Common diagnosis / Types of patients All ages, compromised multiple comorbidities
- Experience or 'Must have' skills Must have prior home health experience, unless otherwise specified.
- Common duties & job responsibilities Regular PT work, completion of documentation within 1 day of patient visit
- EMR HCHB (Home Care Home Base)
- Manager/Supervisor available 24/7? YES
- Have Rehab Aides: PTAs? Yes- COTAs
- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) M-F - 8a-5p
- Specialty: Home Health PT
