Candidate Pay Rate: $95 - $66.5 per hour

Roles & Responsibilities
- Conduct comprehensive patient assessments to develop individualized home health plans
- Implement therapeutic exercises and interventions to improve mobility and function
- Educate patients and families on safe techniques and home exercise programs
- Collaborate with physicians and interdisciplinary care team to optimize patient outcomes
- Document patient progress accurately and maintain compliance with home health regulations
Qualifications
- Active Physical Therapist (PT) license in the state of Idaho
- Home Health experience required
- BLS certification required
- Strong clinical and communication skills
Knowledge & Skills
- Excellent clinical assessment and problem-solving skills
- Clear communication with patients, families, and the care team
- Knowledge of home health safety protocols
- Ability to adapt treatment plans to a home environment
- Strong documentation skills
