Location: Santa Maria, California

Start Date: 06-13-2026

End Date: 09-12-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; weekend rotation every 3rd weekend

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services to patients in their homes
Develop and implement individualized treatment plans based on patient needs
Complete OASIS assessments and documentation as required by government regulations
Collaborate with interdisciplinary team including OT, SLP, and nursing staff
Educate patients and families on therapeutic exercises and home safety
Maintain accurate patient records using Cerner EMR system

Qualifications

Current Physical Therapist license in California
Minimum one year of experience as a Physical Therapist
BLS certification required
Experience with OASIS documentation required
Home health experience preferred but not required
Valid driver's license and reliable transportation

Education

Graduate of an accredited Physical Therapy program

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Proficiency with electronic medical records, especially Cerner
Knowledge of orthopedic, stroke, joint, and spinal injury rehabilitation
Ability to work independently and manage a geographic caseload
