Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide exceptional physical therapy services to home health patients
- Conduct initial evaluations and develop personalized treatment plans
- Perform start-of-care visits for orthopedic and geriatric patients
- Collaborate with interdisciplinary team to ensure comprehensive care
- Document patient progress using HCHB (HomeCare HomeBase) EMR
- Educate patients and families on therapeutic exercises and safety
Qualifications
- Active Physical Therapist license in California
- Current BLS certification
- Minimum 2 years of home health experience preferred, but will consider less
- Experience with HCHB (HomeCare HomeBase) highly preferred
- Ability to travel within Tehama County service area
Knowledge & Skills
- Expertise in orthopedic rehabilitation and geriatric care
- Proficient in home health therapy practices
- Strong communication and patient education skills
- Ability to manage a caseload of 5 visits per day
- Comfortable using tablet for EMR documentation
