Shift Type: Days, Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide exceptional physical therapy services to home health patients
Conduct initial evaluations and develop personalized treatment plans
Perform start-of-care visits for orthopedic and geriatric patients
Collaborate with interdisciplinary team to ensure comprehensive care
Document patient progress using HCHB (HomeCare HomeBase) EMR
Educate patients and families on therapeutic exercises and safety

Qualifications

Active Physical Therapist license in California
Current BLS certification
Minimum 2 years of home health experience preferred, but will consider less
Experience with HCHB (HomeCare HomeBase) highly preferred
Ability to travel within Tehama County service area

Knowledge & Skills

Expertise in orthopedic rehabilitation and geriatric care
Proficient in home health therapy practices
Strong communication and patient education skills
Ability to manage a caseload of 5 visits per day
Comfortable using tablet for EMR documentation
