Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide one-on-one physical therapy services to patients in their homes
Develop and implement individualized treatment plans focused on restoring mobility and function
Conduct Start of Care (SOC) visits for orthopedic patients when nursing is not ordered
Collaborate with the care team including RNs, OTs, and SLPs to ensure comprehensive care
Educate patients and families on therapeutic exercises and fall prevention
Document patient progress accurately using HomeCare HomeBase (HCHB) EMR

Qualifications

Active California Physical Therapist license in good standing
BLS certification from the American Heart Association
Minimum 1 year of Home Health experience (strong candidates with less experience will be considered)
Experience with HCHB EMR preferred
Ability to travel up to 40 miles per day within Tehama County

Knowledge & Skills

Expertise in orthopedic rehabilitation and geriatric care
Strong clinical assessment and problem-solving skills
Excellent communication and patient education skills
Ability to work independently and manage a daily caseload of 5 visits
Proficient in HomeCare HomeBase (HCHB) and tablet-based documentation
