Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekend shifts

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform comprehensive patient assessments including start of care, recertifications, and evaluations
Develop and implement individualized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
Provide skilled physical therapy services in a home health setting
Collaborate with clinical managers and educators for guidance and support
Document patient progress and maintain accurate records

Qualifications

Current Physical Therapist (PT) license in Kentucky
BLS certification required
New graduates encouraged to apply; experienced candidates preferred
Previous home health experience a plus but not required

Knowledge & Skills

Excellent clinical assessment and problem-solving skills
Ability to work independently and manage a varied caseload
Strong communication and interpersonal skills
Familiarity with home health regulations and documentation
