Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Conduct comprehensive Oasis Assessments and Start of Care for home health patients
- Provide skilled physical therapy to elderly patients recovering from falls and fractures
- Manage an average of 4-6 patient visits daily using a point system
- Accurately document patient visits and track time using a tablet
- Collaborate with the interdisciplinary team to create personalized care plans
Qualifications
- Active Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care is required
- Home health experience preferred
- Strong clinical reasoning and patient assessment skills
Knowledge & Skills
- Knowledge of Oasis documentation and home health regulations
- Skills in treating geriatric patients with orthopedic conditions
- Ability to manage caseload efficiently and independently
- Excellent communication and teamwork abilities
