Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Conduct comprehensive Oasis Assessments for home health patients
- Manage Start of Care cases, developing individualized treatment plans
- Provide skilled physical therapy interventions to improve mobility and function
- Collaborate with interdisciplinary team to ensure holistic patient care
- Educate patients and families on home exercise programs and safety
- Accurately document patient progress using a tablet-based system
Qualifications
- Active Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care preferred
- Comfortable with home health settings and driving between patient visits
- Excellent communication and documentation skills
Education
- Graduate of an accredited Physical Therapy program (Masters or Doctorate)
Knowledge & Skills
- Skilled in geriatric rehabilitation, especially post-fall and fracture recovery
- Proficient in using EMR and point-of-care technology
- Strong clinical reasoning and patient education abilities
- Knowledge of Medicare home health regulations
