Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care assessments and develop individualized care plans
Perform recertifications and resumption of care visits for home health patients
Provide skilled physical therapy interventions in patients' homes
Collaborate with interdisciplinary team to ensure optimal patient outcomes
Complete timely and accurate documentation using OASIS

Qualifications

Active Physical Therapy license in North Dakota (or compact eligible)
BLS certification from the American Heart Association
Current driver's license and auto insurance
Home health experience strongly preferred, including OASIS training
Willingness to travel within a 50-mile radius (no candidates from CommonSpirit facilities in past 12 months)

Knowledge & Skills

Patient assessment and evaluation
Home health and hospice care principles
OASIS documentation
Time management and self-direction
Strong interpersonal and communication skills
