Roles & Responsibilities

• Provide high-quality home health physical therapy including start-of-care, recertifications, and resumption-of-care visits
• Deliver regular patient visits (typically 5–6/day when travel is minimal) with thoughtful assessment and intervention
• Perform admissions and add-on assessments as needed and adjust caseload priorities accordingly
• Document thoroughly and accurately in the agency charting system and OASIS when applicable
• Collaborate with clinical coordinators, directors, and sister-agency PTs for consults and complex cases
• Conduct one-time safety evaluations or teaching visits in hospice situations when requested
• Represent the agency professionally in the community and maintain patient-centered care

Qualifications

• Active North Dakota Physical Therapy license (ND Physical Therapy License) required
• BLS certification required
• Minimum: 1 year of relevant experience; Home Health experience preferred
• OASIS training preferred — experience with home health documentation a plus
• Valid Driver's License and current Auto Insurance required (travel to patient homes)
• Ability to complete RTO up front — no additional RTO approvals after submission
• Candidates must not have been employed full-time, part-time, or PRN with CommonSpirit (including CHI or Dignity) within the past 12 months

Knowledge & Skills

• Skilled in home health assessments, start-of-care and recertification processes
• Strong clinical reasoning for individualized treatment planning in home settings
• Comfortable with OASIS documentation and electronic charting systems
• Excellent time management and route planning for efficient daily visits
• Clear communication and patient/family education skills
• Ability to collaborate effectively with interdisciplinary teams and remote clinical resources
