Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; 25% local travel between patient homes

Hours per Week: 40

Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Provide compassionate, patient-centered physical therapy in the comfort of patients' homes
Develop and implement individualized treatment plans to help patients regain mobility and independence
Educate patients and their families on exercises and techniques to promote recovery and prevent injury
Collaborate with a multidisciplinary team to ensure coordinated, high-quality care
Document patient progress accurately and efficiently to track outcomes and adjust treatments

Qualifications

Current California Physical Therapist license in good standing
BLS certification from the American Heart Association
Previous home health experience is a plus, but new graduates with strong clinical rotations are encouraged to apply
Valid driver's license and reliable transportation for local travel between patient homes
Ability to work independently and manage a diverse caseload with time management skills

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Proficiency in electronic medical records and documentation
Flexibility to adapt to different home environments and patient needs
Time management and organizational skills to handle a dynamic schedule
