Roles & Responsibilities

• Assist physical therapists in delivering patient care and rehabilitation services
• Develop and implement individualized rehab plans for residents
• Monitor and document patient progress and outcomes
• Collaborate with interdisciplinary team to optimize patient recovery
• Educate patients and families on exercises and home care techniques

Qualifications

• Licensed Physical Therapist Assistant
• CPR certification
• Strong communication and teamwork skills
• 1+ year experience in a skilled nursing or rehabilitation setting preferred

• Patient assessment and evaluation
• Rehabilitation modalities and equipment use
• Documentation and charting
• Interdisciplinary collaboration
• Patient education
