Roles & Responsibilities

• Assist patients with therapeutic exercises and mobility training
• Collaborate with physical therapists to develop individualized care plans
• Document patient progress and maintain accurate records
• Support patients in daily living activities to promote independence

Qualifications

• Valid Physical Therapist Assistant license in Wisconsin
• Minimum 1 year of experience in a skilled nursing facility or similar setting
• Strong communication and teamwork skills
• Commitment to patient-centered care

Knowledge & Skills

• Knowledge of therapeutic exercise techniques
• Understanding of patient safety protocols
• Proficiency in documentation software
• Ability to adapt to diverse patient needs
