Roles & Responsibilities

• Implement treatment plans developed by the Physical Therapist to help residents improve mobility and function
• Assist patients with therapeutic exercises, gait training, and balance activities
• Document patient progress and adjust interventions under the guidance of the supervising PT
• Educate patients and their families on exercises and safety strategies
• Collaborate with the rehab team to ensure comprehensive care
• Promote independence and quality of life for every resident

Qualifications

• Active Physical Therapist Assistant (PTA) license required
• New graduates are welcome to apply
• Prior SNF or long-term care experience is a plus, but not required

Knowledge & Skills

• Knowledge of rehabilitation techniques and therapeutic exercises
• Understanding of skilled nursing facility care
• Strong communication and interpersonal skills
• Ability to work collaboratively with a rehab team
• Compassionate and patient-centered approach
• Documentation and EHR skills
