Roles & Responsibilities

• Assess and evaluate patients’ physical conditions to develop individualized treatment plans
• Deliver therapeutic exercises, manual therapy, and modalities to improve mobility and reduce pain
• Document patient progress and adjust interventions as needed
• Collaborate with nurses, physicians, and occupational therapists to coordinate comprehensive care
• Educate patients and families on home exercise programs and injury prevention

Qualifications

• Licensed Physical Therapist (PT) with a valid Pennsylvania license
• Minimum of 1 year of experience in a skilled nursing or rehabilitation setting
• Strong communication and documentation skills
• Ability to work independently and as part of a multidisciplinary team
