Roles & Responsibilities

• Provide therapeutic care to residents in an assisted living setting
• Develop individualized treatment plans and monitor progress
• Collaborate with interdisciplinary team to optimize patient outcomes
• Maintain accurate documentation and comply with regulatory standards
• Educate patients and families on exercises and self-care techniques
• Assist with mobility and functional activities to promote independence

Qualifications

• Valid Physical Therapist license in Wisconsin
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Experience in assisted living or long‑term care preferred

Knowledge & Skills

• Therapeutic exercise prescription
• Manual therapy techniques
• Gait training and balance assessment
• Pain management strategies
• Electronic health record documentation
• Patient education and counseling
