Roles & Responsibilities

• Evaluate and treat patients following physician orders and referrals
• Create and update individualized treatment and discharge plans that help patients thrive
• Document care accurately and on time so nothing falls through the cracks
• Supervise COTAs in line with practice acts and facility policies
• Partner with interdisciplinary teams and families to deliver coordinated care
• Keep safety and regulatory standards front and center every day

Qualifications

• Graduate of an accredited Occupational Therapy program (MOT or OTD)
• Active Indiana OT license or the ability to obtain one
• Strong clinical, documentation, and communication skills
• Skilled nursing or rehab experience is a plus
• Compassionate team player ready to make a difference
