Roles & Responsibilities

• Assess patients' functional abilities and develop individualized occupational therapy plans
• Guide clients through therapeutic activities that restore independence in daily living tasks
• Collaborate with nursing, physical therapy, and medical staff to coordinate comprehensive rehab care
• Document progress, adjust treatment plans, and educate patients and families on home exercise programs
• Participate in interdisciplinary team meetings to ensure optimal patient outcomes

Qualifications

• Current Vermont Occupational Therapy license (or eligibility for licensure)
• Minimum 1 year of experience in skilled nursing or rehabilitation settings (new graduates are welcome to apply)
• Strong communication and teamwork skills
• Commitment to patient‑centered care and continuous learning

Knowledge & Skills

• Knowledge of functional assessment tools and adaptive equipment
• Ability to design and implement evidence‑based therapeutic interventions
• Proficiency in electronic health record documentation
• Understanding of Medicare, Medicaid, and private payer regulations
