Roles & Responsibilities

• Conduct occupational therapy evaluations and revisits for home health patients in their homes
• Manage an average of four patient visits per day, including initial evaluations and follow-up revisits
• Cover a service area including northeastern Sacramento, Folsom, and Woodland in Yolo County
• Collaborate with the interdisciplinary team to provide comprehensive care
• Document patient progress accurately in Cerner charting system

Qualifications

• 2 years of experience in a healthcare setting
• Current California Occupational Therapy license
• Basic Life Support (BLS) certification
• NPI number
• Valid driver's license and proof of automobile insurance
• Home health experience preferred; will consider COTA with home health experience if no OT applicants

• Home health occupational therapy

• Strong communication skills
• Ability to work independently in a home setting
