Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (flexible schedule with majority of work hours during this time)

Hours per Week: 40

Candidate Pay Rate: $57 per hour

Roles & Responsibilities

Conduct comprehensive assessments and develop personalized treatment plans for home health patients
Implement therapeutic interventions to improve daily living skills and functional independence
Collaborate with interdisciplinary care teams to ensure coordinated, patient-centered care
Educate patients and families on home exercise programs and adaptive techniques
Document patient progress using Oasis and Cerner EMR systems
Modify treatment plans based on patient response and goals to optimize outcomes

Qualifications

Current California Occupational Therapy license required
BLS certification from American Heart Association
Minimum 1 year of experience as an Occupational Therapist
Experience with Oasis documentation required
Home health experience preferred
Valid driver's license and reliable transportation for patient visits

Knowledge & Skills

Proficient in Oasis documentation and Cerner EMR
Knowledge of orthopedic and neurological rehabilitation techniques
Strong patient education and communication skills
Excellent time management and organizational abilities
Compassionate and patient-centered approach
