Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and monitor resident condition
Administer medications and treatments safely and accurately
Collaborate with the nursing team to develop and implement care plans
Document patient status, care provided, and any changes in condition
Communicate effectively with patients, families, and interdisciplinary staff
Assist with admissions, transfers, and discharges as needed
Support infection control practices and maintain a safe care environment

Qualifications

Valid LPN license in Pennsylvania or eligibility to obtain one
Current BLS/CPR certification (or ability to obtain upon hire)
Demonstrated competence in medication administration and basic nursing procedures
Strong communication skills and ability to work as part of a team
Reliable, professional attitude with attention to patient safety and documentation

Education

LPN diploma, practical nursing certificate, or equivalent
High school diploma or GED

Knowledge & Skills

Foundational nursing assessment and vital signs monitoring
Medication administration and basic IV or wound care familiarity
Electronic medical record (EMR) charting and documentation
Infection control and standard precautions
Time management and prioritization in a fast-paced care setting
Patient-centered communication and compassionate care
