Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and treatments
• Monitor patient vital signs and condition changes, reporting to RNs and physicians
• Assist residents with daily living activities such as bathing, dressing, and eating
• Document patient care accurately and timely in electronic health records
• Collaborate with interdisciplinary team to ensure optimal patient outcomes
• Educate patients and families on health management and discharge plans

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Maine
• Current BLS certification from the American Heart Association
• At least 6 months of recent LPN experience in a long-term care or skilled nursing setting
• Ability to pass a background check and drug screen
• COVID-19 vaccination or approved exemption per facility requirements

• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Proficient in electronic health record documentation
• Ability to work independently and as part of a team
• Compassionate and patient-focused approach to care
