Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Conduct comprehensive psychosocial assessments and develop care plans for patients in a psychiatric setting
Coordinate care and collaborate with multidisciplinary teams to ensure patient safety and well-being
Perform 5150 evaluations for individuals in mental health crisis, determining need for involuntary hold
Provide crisis intervention, counseling, and discharge planning to support recovery
Maintain accurate documentation and comply with facility and regulatory standards
Advocate for patients and connect them with community resources for ongoing support

Qualifications

Active California LCSW license required
BLS certification from AHA or equivalent required
Must be 5150 certified in California (completion of training program and passing exam with 80%)
Prior experience with Dignity Health or CommonSpirit facilities in CA is mandatory
Minimum of 1 year of social work experience in a psychiatric setting preferred
Strong interpersonal and communication skills to work effectively with patients, families, and staff

Knowledge & Skills

Expert knowledge of psychiatric assessment and 5150 hold procedures
Understanding of care coordination and discharge planning principles
Proficiency in electronic medical records and documentation
Cultural competency and ability to work with diverse populations
Crisis intervention and de-escalation techniques
