• Virginia State Pharmacist license required

• complex patients - Trauma/Burn/Critical care, peds, cardiac surgery, ECMO
• Dose Edge IV Experience
• Pyxis Experience
• Meditech
• Vigilanz
• Must have :
• Hospital
• experience > 2years
• Advanced skills needed: Vancomycin AUC dosing
• experience, Aminoglycoside dosing, Renal dosing, warfarin, enoxaparin,
• electrolyte protocols, heparin, DOAC
• Procedures performed on unit: IV
• verification, Medication verification, IV compounding as needed
• Certifications Required: Virginia State Pharmacist license required
