  • Must have :
  • Hospital

  • Advanced skills needed: Vancomycin AUC dosing
  • experience, Aminoglycoside dosing, Renal dosing, warfarin, enoxaparin,
  • electrolyte protocols, heparin, DOAC
  • Procedures performed on unit: IV
  • verification, Medication verification, IV compounding as needed
  • Certifications Required: Virginia State Pharmacist license required
  • Dress Code : (Required uniform)
