Location: Grand Junction, CO

Start Date: 06-29-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating on-call and weekend coverage

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide compassionate, holistic end-of-life care to patients in their homes or facilities
Lead the interdisciplinary team to develop and implement personalized care plans
Teach and empower patients and families to manage symptoms and navigate the journey
Conduct thorough clinical assessments and apply critical thinking to complex situations
Collaborate with physicians, social workers, and chaplains to ensure comfort and dignity
Document patient progress and coordinate care across settings

Qualifications

Active Registered Nurse (RN) license in Colorado or compact state
Minimum 2 years of nursing experience; hospice or home health experience preferred
Excellent assessment, communication, and critical-thinking skills
Ability to work independently and as part of a multidisciplinary team
Valid driver's license and reliable transportation

Knowledge & Skills

Expertise in symptom management and palliative care
Comfort with teaching and supporting families through grief and loss
Proficiency in electronic medical records (EMR) systems
Strong organizational and time-management abilities
Flexibility to adapt to changing patient needs and schedules
