Roles & Responsibilities
- Evaluate and assess residents' speech, language, cognitive-communication, and swallowing function
- Develop individualized, evidence-based treatment plans to improve communication and safety
- Deliver skilled therapy interventions and adapt techniques to each resident's goals
- Collaborate closely with nursing, therapy colleagues, and family members to coordinate care
- Document progress and treatment clearly and timely to meet facility and regulatory standards
- Educate residents and caregivers on strategies to maximize communication and swallowing safety
Qualifications
- Certificate of Clinical Competence (CCC-SLP) required
- 1-3 years of clinical experience (as listed in the job details)
- Experience working in a skilled nursing facility (SNF) setting preferred
- Strong clinical assessment and documentation skills
- Excellent communication and teamwork skills; comfortable collaborating in an interdisciplinary setting
- Available for daytime schedules as required by the facility
Knowledge & Skills
- Skilled in dysphagia screening and management
- Experience with speech, language, and cognitive-communication assessments
- Proficient clinical documentation and care planning
- Ability to provide patient- and family-centered education
- Comfort working within EMR systems and following facility protocols
