Roles & Responsibilities

• Provide expert pediatric inpatient pharmacy care including pharmacotherapy management and dosing decisions
• Prepare and compound sterile and non-sterile IV medications safely using sterile technique
• Verify and dispense medications via hospital medication systems (Pyxis), ensuring accuracy for neonates through adolescents
• Collaborate with physicians, nurses, and interdisciplinary teams to optimize medication therapy and patient outcomes
• Monitor therapeutic drug levels, adjust dosing for renal/hepatic function, and manage high-alert medications
• Support oncology/TPN needs when required — e.g., chemo prep, TPN mixing, IV piggyback and IV push medications
• Document interventions and chart in Epic to maintain continuity of care and clear communication

Qualifications

• Active pharmacist license in the state of Texas (RPh) or eligibility to obtain prior to start
• PharmD preferred (or equivalent pharmacy degree)
• Minimum 1 year hospital pharmacy experience with at least some pediatric exposure
• Comfortable working in a children’s hospital environment and floating between units as needed
• Experience with sterile compounding, IV preparation, TPN/TPN mixers, and chemo handling is highly desirable
• BLS certification preferred; PharmD/RPh credential preferred
• Familiarity with Epic EHR and Pyxis medication dispensing systems is preferred
• Ability to work nights and a block 7 on / 7 off schedule

Education

• PharmD preferred; or equivalent pharmacy degree
• Active RPh (state pharmacist license) required

Knowledge & Skills

• Pediatric dosing and pharmacotherapy management
• Intravenous preparation and sterile compounding techniques
• Chemotherapy handling and packaging antineoplastic agents
• TPN/Hyperalimentation knowledge and mixers for TPN
• IV piggyback and IV push medication administration
• Therapeutic drug monitoring, pharmacokinetics, and renal dosing
• High-alert medication management and warfarin dosing experience
• Familiarity with Epic charting and Pyxis medication dispensing
