Roles & Responsibilities

• Provide hands-on therapeutic treatment and assist patients with mobility, strength, and functional goals
• Deliver quality care within the skilled nursing and assisted living setting, following the established plan of care
• Collaborate closely with the therapy team and onsite Director of Rehab to coordinate patient progress
• Document patient interventions and progress accurately and timely to meet facility and CRG policies
• Maintain required productivity standards (90%) while ensuring individualized, compassionate care
• Adhere to facility protocols including dress code (black scrubs) and CRG policies & procedures

Qualifications

• PTA (Physical Therapist Assistant) experience and skills as listed in the job order
• 1-3 years of relevant experience working in therapy or rehabilitation settings (SNF experience preferred)
• Comfortable working in a 160-bed skilled nursing and assisted living facility environment
• Ability to be onsite and clocked in to receive guaranteed hours
• Willingness to follow CRG policies and meet a 90% productivity requirement

Knowledge & Skills

• Patient assessment and ability to implement therapeutic interventions
• Knowledge of common SNF diagnoses and appropriate rehab strategies
• Strong documentation skills and familiarity with clinical recordkeeping
• Teamwork and communication with interdisciplinary staff
• Time management to balance productivity expectations with quality care
