Location: Sacramento, CA

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: 8-hour shifts, Monday-Friday, with potential for 12-hour night shifts for ED experienced candidates. Minimal time off requests expected.

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Conduct comprehensive discharge planning and utilization review
Coordinate patient care and services
Collaborate with healthcare teams to ensure smooth transitions
Utilize software systems such as Midas, InterCall, Cerner, Curaspan, and Microsoft Office

Qualifications

Minimum 3 years of hospital experience
Acute level care coordination experience
Proficient in Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
RN license in California
Prior CA Dignity/CommonSpirit acute care experience required

Discharge planning and utilization review
Care coordination and case management
Electronic medical records and software systems
Communication and interpersonal skills
