Location: Carmichael, CA
Start Date: 07-07-2026
End Date: 10-06-2026
Contract Type: Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, rotating weekends and holidays
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Develop daily patient lists and review patient charts to assess needs
- Open goals of care discussions and educate patients on disease process
- Facilitate advance directives and POLST conversations
- Hold family meetings and provide end-of-life discussions and support
- Attend daily rounds and collaborate with physicians, NPs, and RNs
- Carry a case load of 5-6 new patients and 3-4 follow-up visits daily
Qualifications
- Active RN license in California
- BLS certification required
- ELNEC certification preferred
- Minimum 2-5 years recent experience in palliative care and/or hospice setting
- Acute care experience required
- ICU or oncology experience a plus
Knowledge & Skills
- Strong clinical thinking and communication skills
- Proficiency with Cerner EMR
- Excellent organizational and flexibility skills
- Desire to serve patients, families, and team members
