Contract Type: Contract
Shift Type: 8-hour shifts, Monday-Friday, with potential for 12-hour night shifts for ED experienced candidates. Minimal time off requests preferred.
Hours per Week: 40
Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Provide acute level care coordination and case management services
- Utilize Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
- Conduct discharge planning and utilization review
- Manage patient assignments with a 1:20 ratio
- Collaborate with multidisciplinary teams to ensure smooth patient care transitions
Qualifications
- Minimum 3 years of hospital experience
- Acute level care coordination experience
- Proficient in Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
- Experience with discharge planning and utilization review
- RN license required
