Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Hours per Week: 40

Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Coordinate patient care across inpatient nursing units, ensuring timely and safe discharges
Perform utilization reviews using InterQual and MCG criteria to ensure appropriate level of care
Collaborate with multidisciplinary teams to develop comprehensive discharge plans
Educate patients and families on post-discharge care and community resources
Maintain accurate and thorough documentation in EPIC
Adapt quickly in a fast-paced environment to manage varying patient needs

Qualifications

Active RN license in Nebraska or compact state
BLS certification required
Minimum 1-2 years of acute care case management experience
Proficiency with EPIC, InterQual, and MCG criteria
Strong communication, time management, and problem-solving skills
