Roles & Responsibilities

• Provide compassionate physical therapy services to home health patients in their residences
• Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
• Collaborate with a multidisciplinary team to ensure coordinated care
• Document patient progress accurately using HomeCare/HomeBase EMR system
• Travel within a 60-mile radius of Valley City to deliver care in patients' homes
• Educate patients and families on exercises and strategies to improve mobility and independence

Qualifications

• Active North Dakota Physical Therapy license required
• Current BLS certification required
• Minimum 1 year of experience as a Physical Therapist, with home health experience preferred
• Valid driver's license and auto insurance is a must
• Willingness to travel up to 60 miles for patient visits

Knowledge & Skills

• Strong clinical skills in physical therapy assessment and treatment
• Experience with home health and hospice patient populations
• Proficiency with HomeCare/HomeBase or similar EMR systems
• Excellent communication and patient education skills
• Ability to work independently without direct supervision
