Physical Therapist - Home Health
Location: Santa Maria, California
Start Date: 06-13-2026
End Date: 09-12-2026
Contract Type: Contract
Shift Type: Flexible schedule, majority of work hours Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy services to patients in their homes
- Develop and implement individualized treatment plans to improve mobility and function
- Collaborate with interdisciplinary team members for coordinated care
- Document patient progress using Oasis and Cerner EMR
- Educate patients and families on home exercise programs and safety
Qualifications
- Active California Physical Therapy license
- BLS certification required
- Minimum one year of PT experience (home health experience preferred but not required)
- Experience with Oasis documentation required
- Ability to work independently and manage a geographic caseload
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Strong clinical assessment and treatment planning skills
- Proficiency with electronic medical records (Cerner preferred)
- Excellent communication and patient education abilities
- Time management and organizational skills to handle a flexible schedule
