Shift Type: Days, Monday-Friday, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide home health physical therapy services to a diverse patient population including orthopedic joint replacements and geriatric patients with CHF, COPD
Conduct SOC visits and develop individualized treatment plans
Collaborate with an interdisciplinary team including RNs and aides
Document patient care in HCHB (HomeCare HomeBase) electronic medical record
Maintain productivity of approximately 5 visits per day within Tehama County service area

Qualifications

Active California Physical Therapy license
BLS certification required
Minimum 1 year of Home Health experience (new grads considered with home health experience)
Experience with HCHB (HomeCare HomeBase) preferred
Valid driver's license and reliable transportation

Knowledge & Skills

Strong clinical skills in orthopedics and geriatrics
Excellent communication and patient education abilities
Time management and self-direction in a home health setting
Comfortable with tablet-based EMR and teletherapy as needed
