Contract Type: Travel Contract

Shift Type: Monday-Friday, days (flexible scheduling)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments in their homes
Develop and implement individualized physical therapy treatment plans
Provide skilled therapy services focusing on orthopedic and geriatric patients
Collaborate with the interdisciplinary care team including RNs and aides
Document patient progress accurately in HomeCare HomeBase (HCHB) EMR
Educate patients and caregivers on home exercise programs and safety

Qualifications

Current California Physical Therapy license in good standing
BLS certification (American Heart Association) required
Home Health experience preferred; will consider travelers with less than 2 years but must have Home Health experience
Comfortable with SOC visits, especially for ortho patients when nursing not ordered
Reliable transportation and willingness to travel within Tehama County service area

Knowledge & Skills

Proficiency with HomeCare HomeBase (HCHB) EMR system
Strong clinical assessment and critical thinking skills
Ability to manage a daily caseload of 5 visits
Excellent communication and patient education skills
Compassionate, patient-centered approach to care
