Location: Louisville, KY

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with rotating weekend shifts and holiday rotation

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct initial evaluations and ongoing assessments for home health patients
Develop and implement individualized physical therapy treatment plans
Perform start of care, resumption of care, recertifications, and discharges
Provide skilled physical therapy interventions in patients' homes
Collaborate with clinical manager and healthcare team to ensure quality care
Document patient progress and maintain accurate clinical records

Qualifications

Active Physical Therapy license in Kentucky
BLS certification (American Heart Association)
Minimum 1 year of home health experience preferred
Strong clinical skills and ability to work independently
Valid driver's license and reliable transportation

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Excellent communication and interpersonal skills
Ability to manage a caseload and prioritize visits
Skills in treating diverse patient populations including CHF, ortho, and pneumonia
Proficiency with electronic medical records and provided tablet/phone
