Location: Kendallville, Indiana

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Conduct Oasis Assessments and Start of Care visits for elderly patients with falls and fractures
- Develop and implement personalized physical therapy plans to improve mobility and independence
- Collaborate with interdisciplinary teams to ensure holistic patient care
- Document patient progress accurately using a tablet in real-time
- Travel to patient homes across four counties (Noble, Lagrange, Steuben, DeKalb)
Qualifications
- Active Physical Therapist license in Indiana
- Experience with Oasis Assessments and Start of Care required
- Comfortable working in rural and mid-sized town settings
- Must have reliable transportation and valid driver's license
Knowledge & Skills
- Strong clinical assessment and critical thinking skills
- Excellent communication and teamwork abilities
- Proficient in using electronic health records and tablet devices
- Ability to manage a caseload of 4-6 patient visits daily
