Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct comprehensive home health assessments and develop individualized treatment plans
Perform start of care, recertifications, and resumption of care visits
Collaborate with interdisciplinary team to ensure coordinated patient care
Document patient progress and maintain accurate clinical records
Educate patients and families on treatment plans and home exercises
Manage a caseload of 5-6 patients per day with focus on quality outcomes

Qualifications

Must possess an active North Dakota Physical Therapy license
Current BLS certification from the American Heart Association
Valid driver's license and reliable auto insurance
Preferred: Prior Home Health experience and OASIS training
Willingness to travel within a 50-mile radius
Ability to work independently and manage a daily caseload
