Medical Social Worker - Inpatient
Location: Seattle, WA
Start Date: 04-20-2026
End Date: 07-20-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day). Minimum 2 weekend days per month (typically one Saturday and one Sunday, not the same weekend). May float to other Swedish campuses (Issaquah, First Hill, Ballard) as needed.
Hours per Week: 40
Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Perform thorough biopsychosocial assessments and develop patient-centered plans of care
- Coordinate multidisciplinary care and transition planning for neuro and cardiac patients
- Lead discharge planning including DME ordering and arranging home care services
- Provide crisis intervention and high-risk patient safety support when needed
- Document clearly and comprehensively in Epic to facilitate continuity of care
- Facilitate referrals to rehab, long-term care facilities, and community resources
- Participate in care conferences and collaborate with PT, respiratory, pharmacy and other teams
- Support patients and families with advance directives, guardianship, and power of attorney issues
Qualifications
- BLS and LSWAIC required
- Epic experience required (charting/documentation proficiency)
- Minimum 1 year inpatient/acute care case management experience
- Hospital inpatient experience preferred; comprehensive stroke center experience a plus
- Willingness to float to other Swedish campuses as needed
- Able to provide weekend coverage as required (minimum 2 weekend days/month)
- First-time travelers accepted; travel experience is not required
Education
- MSW preferred
Knowledge & Skills
- Biopsychosocial assessment and development of individualized care plans
- Care coordination and discharge planning in an acute care setting
- Crisis intervention and high-risk patient management
- Knowledge of Medicaid, Medicare and community programs (AFDC, food stamps, HCBS, etc.)
- Familiarity with adult and child protective services, guardianship and POA processes
- Cultural competence and working with diverse patient populations
- Experience with DME ordering and prior authorization processes
