Roles & Responsibilities
- Provide compassionate direct patient care, delivering daily needs with dignity
- Administer medications and treatments per physician orders and facility protocols
- Accurately document patient care, observations, and changes in condition
- Monitor vital signs and report clinically significant changes promptly
- Assist residents with activities of daily living and supportive nursing tasks
- Collaborate with RNs and the interdisciplinary team to implement care plans
- Support safe transitions of care and communicate clearly with families and providers
Qualifications
- Current, active LPN license (Licensed Practical Nurse) — required
- Graduate of an accredited practical nursing or LPN program
- Basic Life Support (BLS/CPR) certification preferred or will be required upon hire
- Strong communication, observational, and documentation skills
- Compassionate approach and ability to work well within a care team
Education
- Diploma or certificate from an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Medication administration and common nursing procedures
- Patient assessment and basic clinical monitoring
- Charting and electronic health record (EHR) documentation
- Infection control practices and safety protocols
- Time management and prioritization in a busy care setting
- Collaborative teamwork and clear communication with patients and families
