Candidate Pay Rate: $32 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents, promoting dignity and comfort
Administer medications and treatments safely and accurately according to policy
Assess residents regularly and report condition changes to the interdisciplinary care team
Document care thoroughly and timely in the electronic health record
Collaborate with families and staff to create person-centered care plans
Assist with admissions, discharges, and transfers to ensure smooth transitions

Qualifications

Active LPN license in Vermont or eligible to obtain Vermont licensure
Current BLS/CPR certification (or willingness to obtain prior to start)
Experience in long-term care, skilled nursing, or a related setting preferred
Ability to pass background checks and required health screenings
Strong communication skills and a patient-centered approach

Education

Graduate of an accredited Practical Nursing program

Knowledge & Skills

Medication administration and basic clinical procedures
Wound care fundamentals and ADL assistance
Accurate charting and familiarity with electronic health records
Effective teamwork, time management, and prioritization
Patient assessment, infection control, and safety practices
