Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care setting
- Administer medications and treatments accurately and document per policy
- Perform thorough patient assessments and escalate changes to the care team
- Collaborate with RNs, CNAs and interdisciplinary team members to deliver individualized care plans
- Maintain clear, timely charting and ensure regulatory compliance with documentation
- Support residents with activities of daily living while preserving dignity and independence
Qualifications
- Active LPN license in Pennsylvania (required)
- Completion of an accredited Practical Nursing program or equivalent
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable administering medications and performing routine nursing procedures
- Strong communication skills and a collaborative mindset
- Ability to pass background checks and meet facility onboarding requirements
Education
- Diploma or certificate from an accredited Practical Nursing / LPN program
Knowledge & Skills
- Patient assessment and monitoring
- Medication administration and documentation
- Care planning and execution in a long-term care environment
- Familiarity with electronic medical records and standard nursing documentation
- Infection control practices and resident safety protocols
- Teamwork, time management, and compassionate bedside manner
