Roles & Responsibilities

• Conduct comprehensive psychosocial assessments to understand patients' needs and strengths
• Develop individualized treatment plans that are realistic, evidence-based, and person-centered
• Deliver evidence-based therapeutic interventions across the lifespan to address depression, anxiety, trauma, substance use, and adjustment concerns
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues to ensure coordinated whole-person care
• Provide care coordination and referrals to community resources to support treatment goals
• Maintain timely, accurate clinical documentation that supports quality care and continuity
• Serve as an advocate for underserved populations in a rural healthcare setting

Qualifications

• Active LISW license in Iowa (required)
• Master's degree in Social Work from an accredited program (required)
• Minimum of 2 years of clinical experience preferred
• Experience working in rural or medically underserved settings is a strong plus
• Demonstrated skills in psychosocial assessment, treatment planning, and evidence-based interventions
• Strong interpersonal, organizational, and documentation skills

Education

• Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

• Psychosocial assessment and clinical formulation
• Treatment planning and progress evaluation
• Evidence-based therapeutic interventions (e.g., CBT, trauma-informed approaches)
• Care coordination and interdisciplinary collaboration
• Clinical documentation best practices
• Working knowledge of behavioral health needs across the lifespan
• Comfort serving rural and underserved populations with sensitivity and cultural competence
