Roles & Responsibilities
- Provide compassionate direct patient care to promote comfort and recovery
- Administer medications and treatments safely and accurately according to orders
- Monitor patient status and promptly report changes to the interdisciplinary team
- Document patient observations, interventions, and outcomes clearly and timely
- Assist with admissions, transfers, and discharges with professionalism and sensitivity
- Support patient and family education to encourage informed care decisions
- Collaborate with RN and provider staff to maintain continuity and quality of care
Qualifications
- Current LPN licensure in good standing (state licensure required)
- Completion of an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification or willingness to obtain
- Prior experience in a clinical setting preferred but new grads may be considered
- Strong communication skills and ability to work as part of a multidisciplinary team
- Ability to follow facility policies and maintain patient confidentiality
Education
- Graduation from an accredited Practical Nursing program (LPN)
Knowledge & Skills
- Medication administration and documentation best practices
- Fundamental nursing assessments and vital signs monitoring
- Wound care basics and assistance with ADLs
- Electronic medical record charting and handoff communication
- Infection control, safety protocols, and patient advocacy
- Time management and prioritization in a fast-paced setting
