Candidate Pay Rate: $36 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered bedside care to assigned residents
- Administer medications and treatments accurately and safely per orders
- Assess, monitor and document vital signs, symptoms and patient responses
- Collaborate with nurses and interdisciplinary team to deliver coordinated care
- Assist with activities of daily living while promoting patient dignity and independence
- Communicate clearly with patients, families and care team about care plans
- Follow facility policies, safety protocols and infection control practices
Qualifications
- Active Licensed Practical Nurse (LPN) license (required)
- Completion of an accredited LPN/Vocational nursing program
- Current Basic Life Support (BLS) or CPR certification preferred or required per facility
- Previous clinical experience in long-term care or similar setting is a plus
- Strong communication, teamwork and documentation skills
- Ability to pass facility background and health screenings
Education
- Graduation from an accredited LPN/Vocational Nursing program
Knowledge & Skills
- Medication administration and basic clinical assessments
- Wound care and ADL support in a long-term care environment
- Accurate charting and electronic documentation practices
- Infection control and safety procedures
- Effective patient and family communication and education
- Team collaboration and time management under a per diem model
